  1. Employee washed hands for four seconds prior to making food. Employee rewashed hands for 20 seconds.
  2. Employee placed hands in sweater before slicing tomatoes. Employee washed hands for 20 seconds.
  3. Knives, blenders and cutting boards not properly washed, rinsed or sanitized. No sanitizer present at the establishment. 
  4. Sliced tomatoes, salad mix, cheddar cheese, mozzarella, chicken, turkey, ham and parmesan cheese held at improper temperature. Discarded. 
  5. Chicken expired. Discarded.
  6. Handwashing sink not connected to drain line.

  1. Employee wiped gloves on apron then handled food on grill. Employee washed hands and switched gloves.
  2. Employee used bare hands to grab lime from container. Lime discarded, tongs provided.
  3. Chicken stored below raw salmon. Items rearranged.
  4. Chlorine in mechanical warewasher below proper concentration. Primed machine to proper concentration.
  5. Chicken, cooked mushrooms, raw pork and raw salmon held at improper temperature. Moved to freezer to cool.

  1. Employee grabbed lettuce with bare hands and placed on burrito. Lettuce and burrito discarded.
  2. Employee did not sanitize salsa pan. Employee sanitized pan.
  3. Sanitizer in basin below proper concentration. Employee added bleach to the water to reach proper concentration.
  4. Pesticide stored in mop closet. Employee removed pesticide.

  1. Employees drinking drinks without lids or straws. Employees removed all unapproved drinks from kitchen.
  2. Raw chicken stored above raw pork. Items rearranged.
  3. Raw fish, crab, beef ribs, beef brisket, halibut and eggs held at improper temperature.
  4. Sanitizing solution in bucket at improper concentration. Discarded.
